16/07/2026 16/07/2026 BLOWS, Susan In late November 2025 Susan BLOWS sustained an injury to her arm at home. She went on to develop a severe infection, which led to organ failure. Her underlying respiratory conditions affected her ability to withstand the infection and despite maximal treatment, she died at Royal Preston Hospital on 7th December 2025 
16/07/2026 16/07/2026 GALBRAITH, Kathryn Rosemary Kathryn Rosemary GALBRAITH who suffered from a tumour on her brain died at Royal Preston Hospital on the 14th of August 2025 having suffered a stroke following essential shunt revision surgeries on the 5th and 11th of August 2025
16/07/2026 16/07/2026 JAN LALL, Aksar Mrs Aksar JAN LALL died on the 31 March 2026 at Burnley hospital from pneumonia. Her death was accelerated by her underlying frailty and burns she had sustained on the 1st February 2026 when her hot water bottle burst causing injuries to her legs. Mrs LALL received treatment for her burn injuries, however she then developed pneumonia on the 29th March 2026 and despite antibiotic treatment, she deteriorated and died on the 31st March 2026.

16/07/2026 16/07/2026 JESSOP, Philip Philip JESSOP died on 25th March 2026 at 10 Shorrocks Avenue, St Michaels, Preston from an acute cardio respiratory arrest caused by a chest infection and coronary artery disease. His emphysema and recent rib fractures made him susceptible to the chest infection. 
16/07/2026 16/07/2026 RILEY, Eileen Eileen RILEY died at her residential care home in Haslingden on the 28th March 2026 from vascular dementia and old age. Her death was contributed to by a recent fractured neck of femur and right humerus fracture sustained on the 13 March 2026 when Mrs RILEY suffered an unwitnessed fall in her care home. She was admitted to hospital and following surgery she continued to deteriorate. Mrs RILEY was discharged home for end of life care on the 25th March 2026 and died on the 28th March 2026. Mrs RILEY had a number of comorbidities including severe frailty, heart failure and Alzheimer's dementia which also contributed to her death.

23/07/2026 23/07/2026 SIMPSON, Winifred Ann Winifred SIMPSON died from frailty of old age at Lowfield Residential Home in Clitheroe on the 6th March 2026. Mrs SIMPSON was discharged to Lowfield Residential Home from hospital on the 12th June 2025 where she had received surgery for a fractured neck of femur sustained when she fell to the ground in her previous nursing home on the 24th May 2025. The incident on the 24th May 2025 occurred when Mrs SIMPSON fell to the ground as she moved out of the way of another resident who was attempting to push her. The fracture and its subsequent repair accelerated her death. Her death was contributed to by her underlying Alzheimer's dementia. Mrs SIMPSON at the time of the incident in May 2025 was placed on a complex dementia floor of the care home where she should not have been placed; steps to reduce the risk of the incident occurring were not taken; and her fall and subsequent fracture was therefore avoidable. 
27/07/2026 04/08/2026 CLAYTON, Patrick Sean Patrick Sean CLAYTON was found hanged in his cell at HMP Garth on Third of September 2024, having taken steps to end his life. He died on Fourth of September 2024 at Royal Preston Hospital, Preston following withdrawal of treatment. The cause of death was hanging with anxiety and depression as a contributing factor.

28/07/2026 28/08/2026 Nutall Coulthard, Eliza Eliza NUTTALL COULTHARD was found unresponsive in her home and taken to Blackburn hospital where, despite resuscitation efforts, she died on the 23rd October 2022. Post mortem examinations could not determine the cause of her death although it is noted that she was found in a collapsed state in the context of an unsafe sleeping environment. Eliza was put to bed in her cot on the evening of the 22nd October 2022 and when she did not settle, she was brought into the bed to be comforted. She was found unresponsive a few hours later which is when emergency services were called and she was taken to hospital.
29/07/2026 29/07/2026 DEAN, Mary Catherine Mrs Mary Catherine DEAN died on the 27th September 2024 at Banksfield Nursing Home from chest sepsis. Mrs DEAN was discharged to the nursing home the day before, from Preston Hospital where she had been admitted on the 10th September 2024 with shortness of breath following a period of general decline and multiple prior hospital admissions and previous falls. On the 11th September 2024, Mrs DEAN sustained a fractured neck of femur due to an unwitnessed fall within the Emergency Department. She was assessed as being a low risk of falls and was placed in a side room of the ED with the door shut to adhere to barrier nursing as she had a c-difficile infection at the time. Mrs DEAN had delirium and cognitive impairment and could not use the call bell. Following the fall, Mrs DEAN underwent surgical repair of the fracture and was discharged to the nursing home on the 26th September following which she acutely deteriorated and died. She had a number of underlying comorbidities which contributed to her death.
