Toxicology shows he had a high level of carbamazepine which was not prescribed to him, in his system. He also was withdrawing from alcohol and likely suffered a seizure. The seizure and the carbamazepine toxicity likely caused his death and this was contributed to by coronary artery disease and liver disease. |

| 05/08/2026 | 05/08/2026 | WASILEWSKI, Sylwester Robert | Mr Sylwester Robert WASILEWSKI died on the 18th December 2025 at Hawkhurst Nursing Home in Blackburn from the effects of hypoglycaemia. Mr WASILEWSKI had been an inpatient in Blackburn hospital between 13th October 2025 until his discharge to the care home on the 17th December 2025. Mr WASILEWSKI had type 1 diabetes which was controlled with insulin. Whilst a hospital inpatient, a recommendation to reduce his short acting insulin dose had been made but his prescription was not changed. He did not receive a diabetic specialist nursing review before he was discharged. Consequently, upon admission to the care home, he received an excessive dose of his insulin which contributed to his death. He was observed sleeping with an untouched meal near to his bed, around two hours before he was found collapsed. Mr WASILEWSKI was found unresponsive a short time after and despite resuscitation he was pronounced deceased by paramedics. Mr WASILEWSKI has a number of underlying comorbidities which contributed to his death. |

| 12/08/2026 | 12/08/2026 | CLARKSON, Andrew Paul | Andrew CLARKSON died on the 4th November 2024 in St Catherine's Hospice, Preston. Mr CLARKSON died from a hospital acquired pneumonia with cavitation and empyema which he developed following a prolonged hospital admission. Mr CLARKSON was admitted to Preston hospital on the 26th September 2024 following a fall in his supported living home and he was diagnosed with campylobacter which was treated conservatively. He was considered medically optimised for discharge and moved to a ward where he did not receive the necessary nutrition and hydration for a number of days causing him to become very dehydrated and malnourished. He suffered an acute deterioration on the 17th October 2024 and was admitted to the Critical Care Unit however his condition continued to deteriorate and care was orientated to palliation and he was moved to St Catherine's Hospice. His death was contributed to by a number of failures in care and neglect. |

Mr CATHERALL died as a result of bronchopneumonia caused by malignant mesothelioma and metastatic prostatic adenocarcinoma. It has not been possible on the available evidence to determine what caused the mesothelioma. |
| 13/08/2026 | 13/08/2026 | CHARNOCK, Christian Thomas | Christian Thomas CHARNOCK died on 26 November 2025 at his home address. Mr CHARNOCK had a history of suffering with his mental health. Evidence was heard that this resulted in him drinking alcohol which then caused a decline in his physical health including problems with his liver and he became increasingly frail. He was discovered unresponsive in his bedroom when his family checked on him. Toxicological testing revealed Mr CHARNOCK had consumed Methylenedioxymethamphetamine (MDMA) which caused his death. Mr CHARNOCK's reduced liver function increased the risk of a fatal outcome from MDMA |

| 13/08/2026 | 13/08/2026 | HILLMAN, Neil Jack | Mr Neil Jack HILLMAN died on the 17th December 2025 in Blackburn Hospital from complications arising from an angiogram procedure performed on the 12th November 2025 to revascularize the circulation to his left lower limb. Following the index procedure, Mr HILLMAN reattended hospital on the 23rd November 2025 and there was a delay in recognising the infected right groin pseudoaneurysm and taking him to theatre. Mr HILLMAN suffered a major haemorrhage which would have otherwise been avoided and this materially contributed to his death. Mr HILLMAN required thereafter a above knee amputation and subsequently developed a prolonged period of infection. Despite antibiotic treatment he continued to deteriorate and he developed multi-organ failure. His care was orientated to palliation prior to his death. He had underlying comorbidities of diabetes, hypertension and Chronic obstructive pulmonary disease which reduced his physiological reserve further. |

He had been admitted to hospital on the 14th of January 2025 suffering from cholangitis which required an urgent procedure. That was delayed due to his medication, his lung function, and a fractured neck of femur sustained in an unwitnessed fall on the Ward on the 22nd of January 2025. A previous admission to hospital in September 2024 when cholangitis had been suspected had not been followed up with an outpatient appointment or further necessary investigation despite the results of blood tests that had been taken. |
