18/05/2026 19/05/2026 GOUDE, Ian Anthony Ian Anthony GOUDE died on 23 June 2025 at Royal Preston Hospital, Preston in Lancashire. Mr GOUDE was admitted to hospital on 18 June 2025 following showing signs of a stroke. He developed malignant MCA syndrome which causes swelling in the brain and was transferred to another hospital to undergo emergency surgery to relieve pressure caused by the swelling. Despite treatment over the following days, he did not recover. Mr GOUDE was eligible for treatment by thrombectomy which was not available locally at the time of night when he was admitted to hospital, which possibly contributed to his death.

20/05/2026 20/05/2026 NELSON, Stephen Paul Stephen NELSON was found deceased on the 5th January 2026 at his home address in Skelmersdale, Lancashire. Mr NELSON died from high levels of methadone combined with a chest infection on a background of emphysema and chronic obstructive pulmonary disease. He was suffering with severe fatty liver infiltration which also contributed.

29/05/2026 29/05/2026 PARKINSON, Alyson Rhian Alyson Rhian PARKINSON died on the 5th of June 2025 at Royal Blackburn Hospital from necrotising pancreatitis. Following admission to hospital in April 2024 when gall stones were diagnosed a recommended and anticipated referral for a possible cholecystectomy was not pursued as it should have been. Mrs PARKINSON was remitted to hospital on the 30th of April 2025 where her condition became inoperable. Earlier surgical intervention would have offered an opportunity to prevent her death.
