Mrs KIRKLEY was admitted to hospital following a number of falls at the care home where she was resident which resulted in a number of rib fractures. She developed an infection from which she did not recover. Evidence was heard that Mrs KIRKLEY had become increasingly frail and suffered from mixed dementia which contributed to the falls occurring |
| 21/09/2026 | 21/09/2026 | MILLER, James | James MILLER was admitted to Royal Blackburn Hospital on 13 April 2026 with symptoms of severe pneumonia and despite best medical efforts he continued to deteriorate and died there on 14 April 2026. The pneumonia was experienced against pre-existing conditions of occupationally caused diffuse pleural thickening and coronary artery disease, which contributed to his death by making him more susceptible to the effects of the pneumonia. |

| 22/09/2026 | 22/09/2026 | HURTLEY, William | Mr William HURTLEY died on the 17th December 2025 in Preston hospital from chest sepsis and hypertensive heart disease. Mr HURTLEY was admitted to hospital on the 1st December 2026 with a chest infection and heart failure and during his admission it was suspected that he was suffering from a pulmonary embolism. A CT Pulmonary angiogram ruled out a pulmonary embolism but despite this Mr HURTLEY continued to receive a treatment dose of anticoagulant for another two days which caused him to suffer a retroperitoneal bleed. Despite treatment to reverse the effects and confirmation that there was no longer any active bleeding, the haemorrhage affected his physiological reserve and made him more susceptible to infection and contributed to his death. |

Natasha was found submerged in the bath where a sponge was covering the plug and she died from drowning. At the time of her death she was acutely intoxicated. Natasha was experiencing significant challenging personal issues at the time of her death and was being supported by professionals. Her intentions cannot be determined. |
