| 24/08/2026 | 24/08/2026 | BAMBER, Peter | Peter Edward BAMBER died on the 1st September 2025 at the Royal Preston Hospital. Mr BAMBER had a number of health conditions and was admitted to hospital on the 6th August 2025 as an emergency for treatment of a sigmoid volvulus. Mr BAMBER underwent surgery on the 7th August 2025 and initially commenced a slow but steady recovery. In the course of his admission, Mr BAMBER was wrongly prescribed medicine for his diabetes which was given daily instead of as required. On the morning of the 1st of September, Mr BAMBER was given insulin for high blood sugars. He was later found unresponsive and despite blood sugar correction, did not recover. It is not possible to determine what caused Mr BAMBER's death but it was contributed to by a number of pre-existing health conditions, along with his necessary emergency surgery and his diabetes. |

| 25/08/2026 | 25/08/2026 | HUTCHINS, Hayley Rachael | Hayley Rachael HUTCHINS died on the 2nd September 2025 at the Royal Preston Hospital as a consequence of dilated cardiomyopathy. Mrs HUTCHINS had a number of health conditions and had been admitted to the Royal Oldham Hospital in March of 2025 where she underwent surgery for gall bladder issues. Following the surgery, Mrs HUTCHINS required further interventions to resolve a bile duct leak. Mrs HUTCHINS made an initial recovery and was discharged home but on the 21st of June 2025 was taken by ambulance to the Royal Preston Hospital in septic shock. Despite treatment at Royal Preston Hospital to resolve biliary sepsis, she continued to deteriorate and died as a result of her underlying dilated cardiomyopathy. Her death was contributed to by biliary sepsis which hastened her death from underlying cardiac cause. |

Mrs BANKS' death was caused by bleeding as a result of longstanding ulcerative colitis. Her death was contributed to by left pleural epithelioid mesothelioma. |

Mr CLARINGBULL had a complex medical history and was admitted to hospital for vascular issues relating to his foot. Whilst in hospital he was noted to have lung cancer which had spread to other parts of his body. On the 9th March 2026 Mr CLARINGBULL had a fall whilst mobilising to the toilet and sustained a fracture to his neck of femur which was operated upon. He developed pneumonia which, when combined with pre-existing chronic obstructive pulmonary disease and his lung cancer, resulted in respiratory failure and his death. The injuries he sustained in the fall contributed to his death by impacting his already weakened physiological reserve. |

Mr CLARKSON had a complex medical history and a number of health conditions which combined to result in his death by frailty of old age. One of the contributing conditions was caused by occupational exposure to asbestos in the course of his previous working life. |

Evidence was heard that Mr DOYLE would sometimes drink alcohol to excess and use substances on occasion. He died as a result of acute alcohol and mixed drug toxicity, including from prescription medication. His death was impacted upon by a lower tolerance to morphine due to his limited use of morphine in the past |

| 27/08/2026 | 27/08/2026 | HUGHES, Eric | Eric HUGHES died at his home on the 18th of February 2026 from an infection. He had a number of comorbidities and had suffered a fall at his home on the 23rd of November 2025. He was not fit for surgery and was treated with a cast on his leg in an attempt to assist healing. He developed pressure sores which alongside a urinary tract infection were the underlying causes of his death. |

Mrs SHOREMAN suffered a cardiac arrest on the 14th May 2026 having consumed a number of drugs. Paramedics achieved a return of circulation from her heart but she had suffered a significant lack of oxygen to her brain which resulted in her death two days later. |
