| 03/09/2026 | 03/09/2026 | SUNNERS, Ronald | Mr Ronald SUNNERS died on the 10th January 2025 in Preston Hospital from an exacerbation of his chronic obstructive pulmonary disease caused by an influenza A infection. Mr SUNNERS had underlying asthma and heart failure which contributed to a reduction in his functional reserve. During his time in the emergency department Mr SUNNERS was provided with non-invasive ventilation and for a period of time the incorrect tubing was used to connect the mask to the machine, which accelerated his death. |
| 07/09/2026 | 09/09/2026 | ELLIS, Jack Owen | Jack Owen ELLIS died on the 22nd May 2025 in Preston Hospital from complications arising as a result of a malfunctioning ventriculoperitoneal shunt which caused Jack to suffer a prolonged seizure and subsequent catastrophic hypoxic brain injury. Jack required the shunt to manage his hydrocephalus which he developed following neonatal bacterial meningitis. He also developed epilepsy subsequent to the neonatal meningitis which predisposed him to suffer seizures and contributed to his death. |

| 10/09/2026 | 11/09/2026 | GRAHAM, Nevaeh-Rayne June | Nevaeh-Rayne June GRAHAM died on the 9th May 2023 at her home address. Nevaeh-Rayne was born with a number of health conditions and had spent an extended time in the Neonatal Intensive Care Unit. Once home, she required significant medical support from her parents and community nurses. In February 2023 Nevaeh-Rayne suffered with a number of infections and on the 21st of February she was observed to vomit, choke and aspirate. She was taken by ambulance to hospital where aspiration pneumonia was confirmed. On the 22nd of February 2023, Nevaeh-Rayne developed sepsis and deteriorated to the point that she required intubation. She was transferred from the Royal Preston Hospital to the Alder Hey Hospital and then to the Manchester Children's Hospital. In March of 2023, attempts were made extubate her. During the attempt, Nevaeh-Rayne's lung collapsed and she suffered cardiac arrest which resulted in hypoxic ischaemic brain injury. Nevaeh-Rayne developed aspiration pneumonia which was the direct cause of her death. |

Mr COCKING died as a result of suicide by means of hanging despite adequate measures being followed to prevent self harm and suicide. Mr COCKING was discovered in his cell by a prison officer. Help was summoned, life saving treatment was administered but health care professionals and paramedics were unable to resuscitate Mr COCKING. Mr COCKING was pronounced dead at the scene. |

| 14/09/2026 | 14/09/2026 | STEWART, David Lennox | David Lennox STEWART died on the 27th August 2025 at the Royal Blackburn Hospital. Mr STEWART had a complex medical history and prior to his death had a long hospital admission before transferring to an acute mental health ward. The mental health ward was not suitable for a patient with such significant physical health needs. Mr STEWART declined some medical treatments but ward staff did not record observations and escalations were slow. Red flag symptoms of physical deterioration were not recognised as a medical emergency and the bank holiday weekend staffing limited escalation options. Mr STEWART's oral intake was not consistently recorded which contributed to dehydration and the development of ketoacidosis. Mr STEWART died as a consequence of severe euglycemic ketoacidosis. His care on the mental health ward contributed to his severe condition and his death. |

| 15/09/2026 | 16/09/2026 | DEWHURST, Stephen | Stephen DEWHURST died in Blackburn hospital on the 8th August 2025 from pneumonia which he developed during a period of rapidly progressing frailty. Mr DEWHURST was being treated in a mental health hospital for a relapse in his bipolar disorder which coincided with medication changes for his type 2 diabetes. He was transferred to Blackburn hospital in July 2025 due to physical health concerns. His condition deteriorated very quickly and despite treatment he continued to deteriorate in keeping with rapidly progressing frailty on a background of comorbidities including his bipolar disorder, an undiagnosed dementia, type 2 diabetes and acute kidney injury which contributed to his death. |

| 17/09/2026 | 17/09/2026 | CLOUGH, Ronald | On 31 March 2026 Ronald CLOUGH died at Oaklands Nursing Home where he had been discharged for palliative care from Royal Blackburn Hospital, suffering from a pneumonia against a background of old age related frailty, contributed to by coronary artery disease and asbestosis from industrial exposure to asbestos during his working life. |

He had a background of long term, regular amphetamine use which was a likely contributor to left ventricular hypertrophy together with known but untreated high blood pressure. |
