23/07/2026 23/07/2026 SIMPSON, Winifred Ann Winifred SIMPSON died from frailty of old age at Lowfield Residential Home in Clitheroe on the 6th March 2026. Mrs SIMPSON was discharged to Lowfield Residential Home from hospital on the 12th June 2025 where she had received surgery for a fractured neck of femur sustained when she fell to the ground in her previous nursing home on the 24th May 2025. The incident on the 24th May 2025 occurred when Mrs SIMPSON fell to the ground as she moved out of the way of another resident who was attempting to push her. The fracture and its subsequent repair accelerated her death. Her death was contributed to by her underlying Alzheimer's dementia. Mrs SIMPSON at the time of the incident in May 2025 was placed on a complex dementia floor of the care home where she should not have been placed; steps to reduce the risk of the incident occurring were not taken; and her fall and subsequent fracture was therefore avoidable.

29/07/2026 29/07/2026 DEAN, Mary Catherine Mrs Mary Catherine DEAN died on the 27th September 2024 at Banksfield Nursing Home from chest sepsis. Mrs DEAN was discharged to the nursing home the day before, from Preston Hospital where she had been admitted on the 10th September 2024 with shortness of breath following a period of general decline and multiple prior hospital admissions and previous falls. On the 11th September 2024, Mrs DEAN sustained a fractured neck of femur due to an unwitnessed fall within the Emergency Department. She was assessed as being a low risk of falls and was placed in a side room of the ED with the door shut to adhere to barrier nursing as she had a c-difficile infection at the time. Mrs DEAN had delirium and cognitive impairment and could not use the call bell. Following the fall, Mrs DEAN underwent surgical repair of the fracture and was discharged to the nursing home on the 26th September following which she acutely deteriorated and died. She had a number of underlying comorbidities which contributed to her death. 
30/07/2026 30/07/2026 KEARNS, Christopher Craig Mr Christopher Craig KEARNS was found deceased in his home in Preston on the 16th March 2026. Toxicology shows he had a high level of carbamazepine which was not prescribed to him, in his system. He also was withdrawing from alcohol and likely suffered a seizure. The seizure and the carbamazepine toxicity likely caused his death and this was contributed to by coronary artery disease and liver disease.

30/07/2026 30/07/2026 PATEL, Kalpeshbhai Maganbhai Kalpeshbai Maganbhai PATEL was found deceased in the Leeds Liverpool canal in Hoghton, Preston on the 12th February 2026. The circumstances leading to him entering the water are not known. His cause of death was drowning.
