Claiming the gift

Employee should:

Download and print off a long service award claim form (DOCX 140KB).

Alternatively if you are unable to download the form you can contact AskHR by raising a service request from the Helpdesk Module in Oracle Fusion.

Complete the long service award claim form, providing the following details:

  • Description of item.
  • Make, model number, reference number, colour, type, size, etc., as applicable.
  • Price, inclusive of VAT.
  • Name, address and contact details of the shop where the item can be purchased.
  • Service dates with Lancashire County Council.

Send your completed claim form by email to HR.LongServiceAwards@lancashire.gov.uk.

Note: Employees are advised not to purchase the gift themselves, nor to leave a deposit but to go through the following steps.
