Pre-school transition documents

The pre-school transition documents are designed to support children as they move from pre-school to school. These documents allow you to share important information with the child’s new teacher to help ensure a smooth transition.

There are two versions available: one aligned with Development Matters and the other with Birth to 5 Matters.

The pre-school pathway of support is intended for use in the lead-up to children moving from pre-school to school.

There are two versions available: one for pre-school practitioners and another for Reception class teachers. This pathway helps ensure children receive consistent support throughout the transition process.

Transition calendars

The transition calendars have been created to help you view transition as an ongoing process rather than a one-off event.

There are two versions available: one for pre-school and another for Reception and Year 1. Each calendar provides month-by-month suggestions to support a smooth and successful move to the next year group.
