Person centred support plans - day time services - OP0043

Who is this course for?
This one-day training is for staff working in Day time support services in the Older People Service.


Aim(s)
To give an awareness of using person-centred thinking and approaches to help the people you support have more choice and control in their lives. For staff to provide the best support they can in ways that reflects what is important to the person.

Please note that delegates must bring a copy of their own 1 Page Profile to the session (even if it is not fully completed) and also at least one resident's 1 Page Profile but can bring more than one.


Objectives
At the end of the course, the participants will be able to;
• Demonstrate a better understanding of person-centred thinking to enable staff support
• Create better quality support plans
• Identify a range of person-centred tools and how to use them effectively
• Recognise the person at the centre of the support plan
• Identify family members and friends in developing the support plan
• Recognise their own role and how they fit into completing a quality support plan
• Explain the importance of planning for end of life
