Person centred support plan training – community assessment beds (Older People Service) - OP0040

Who is this course for?

This one day training is only for staff working in the Community Assessment Beds in the Older People Service.

Aims

To give an awareness of using person centred thinking and approaches so as to help the people you support have more choice and control in their lives. For staff to provide the best support they can in ways that reflect what is important to the person.

Please note that delegates must bring a copy of their own 1 Page Profile to the session (even if it is not fully completed) and also at least one resident's 1 Page Profile but can bring more than one.

Objectives

At the end of the course, the participants will be able to:

• Demonstrate a better understanding of person centred thinking
• Create better quality support plans
• List a knowledge of a range of person centred tools
• Employ effectively person centred tools
• Relate the person to the centre of the support plan
• Identify family members and friends in developing the support plan
• Recognise their own role and how they fit into completing a quality support plan
• Explain the importance of planning for end of life
