EHC needs assessment requests
How professionals can contact the local authority to request an education, health and care needs assessment (EHCNA).
All education, health and care needs assessment (EHCNA) requests can now be done online. The new system makes everything more streamlined for everyone. The information you send will enter the system straight away. Your documents will be secure and there is a clear record of everything you upload.
Educational settings and professionals will be required to confirm that parent/carer consent has been obtained before submitting a request on a child’s behalf.
Before you start
Some important points before you fill in the form:
- You will be asked to log in to your children and family services account. If you do not have one, it will be created for you as you progress through the form.
- Before you start the application, you should have your supporting documents to hand.
- There are some questions you will be asked that are mandatory and you will not be able to move on to the next stage until you answer them.

Educational psychologist advice
If the local authority agrees to do an education, health and care needs assessment, an educational psychologist (EP) must give advice. Some children or young people will already have recent advice from an EP that may be used. If the local authority thinks more advice is needed, they will ask for permission from the parent, carer, or the young person (if they are 16 and over) before the EP gets involved again.
