Requesting an education, health and care needs assessment
How to contact Lancashire County Council, as the local authority, to request an education, health and care needs assessment (EHCNA).
Who can ask for an EHCNA
The following people can request an EHCNA for a child or young person:
- parents and carers
- young people aged 16 and over
- a person acting on behalf of a school or post-16 institution
Other people can also let the local authority know if they think an assessment might be needed – for example, health or social care professionals.
Making a request online
All education, health and care needs assessment (EHCNA) requests can now be done online. The new system makes everything more streamlined for everyone. The information you send will enter the system straight away. Your documents will be secure and there is a clear record of everything you upload.
Before you start
Some important points before you fill in the form:
- You will be asked to log in to your children and family services account. If you do not have one, it will be created for you as you progress through the form.
- Before you start the application, you should have your supporting documents to hand.
- There are some questions you will be asked that are mandatory and you will not be able to move on to the next stage until you answer them.
