Homecare change requests

Homecare providers can use this form to request changes to existing packages of care:

  • Increase to current package of care (including requesting a review)
  • Suspend package of care
  • Serve notice of package of care
  • CPLI query

If you are requesting an increase to a current package of care and the visit is a one-off of up to 4 hours, continue to submit this additional time on your weekly invoice. This one-off change does not require review by Adults Social Care.

Before you start

We will need to know:

  • personal details, such as the person's name, date of birth and address
  • the person's LAS reference number is known, and next of kin details
  • if consent to the changes has been gained, and if not why
  • changes in the person's circumstance
  • details of the required changes
  • the date that the change is required from and to

Completing the request form

You must have an email address and register for an adult social care account to complete a request.

We will send a security authentication code to your email address when you register.

After you have registered, you can complete and submit the request form, or save and come back to it.

We will email you a different security authentication code each time you sign in to your account. The code is valid for 10 minutes and can only be used once.
