Adult Social Care Portal 

The Adults Portal form can currently be used to Request a Mental Health Act Assessment for those aged 18+. More information about making a Mental Health Act Assessment Request.

Make a Mental Health Act Assessment Request (18+), please use the online Adults Portal request form. For those aged under 18 please visit the AMHP page for the online request form.  

Registering for the portal

To use the Adults Portal to submit a request to Lancashire County Council you will need to register for the Portal to create an account. To register you will need to:

  • Enter your first name and surname
  • Provide an address
  • Enter your email address
  • Create a password

Once you have entered your information you will then be emailed a unique verification code which is valid for 10 minutes. Once the code has expired you will need to begin the process again. The registration process is only complete once the code is entered and accepted.

The code can take up to 5 minutes to arrive, but if it does not arrive after this time, please:

  • Check your spam/junk/other folders for the email. By adding noreply@lancashire.gov.uk to your trusted sender list or contacts it should prevent future verification code emails being sent to these folders.
  • Check with your email provider to make sure emails from noreply@lancashire.gov.uk are not being delayed, blocked or temporarily quarantined.

Logging into the portal

When making future requests, you will be required to login to the portal using the email address and password you used to register.

Every time you login to the Portal, you will be sent a unique verification code which will be emailed to you. The code is valid for 10 minutes and can only be used once.

Please allow time for the verification code to arrive before requesting another one. If you do request another code, the first verification code that was generated will not work.

If you need to reset your password, you can do this through the Portal.

Portal account

One of the benefits of using the portal to submit requests is that you can view your submitted forms from the last 90 days. To find previous requests you need to login to Portal and view them within your account.

If you need to update any of the details linked with your account for future requests, then you can do this within the Portal. Further information on how to update your details is demonstrated in the video.

When you have inputted all the information, attached any supporting documentation and pressed "submit form", then you should receive an onscreen confirmation. You will also receive an email confirmation that the request has been submitted and the request can be viewed, with the unique reference number, in the submitted forms section of your account. 

If you do not receive confirmation that your request has been submitted or you experience any issues when using the Adults Portal, please use the telephone route to complete your request.

Delegation portal

The delegation portal allows a professional to contribute to an assessment through an online form. You login to the delegation portal using the link sent to you in an email.

The delegation portal requires Lancashire County Council to initiate the registration process.  You will then be requested to create an account and login. This works the same way as the Adult Social Care Portal.
