Education, health and care (EHC) plan annual review survey

This survey was designed to be completed at the end, or after, the annual or six monthly (for younger children) review of a child or young person's education, health and care plan.  The review looked at a child or young person's progress towards the outcomes in their education, health and care plan and what they hoped to be able to do in the future.

Why we developed the survey

We developed the survey because Lancashire SEND Partnership wanted to know about whether the support in a child or young person's education, health and care plan has made a difference to their progress and development.

The feedback you gave us is helping us to develop and improve the services that are provided for the children and young people of Lancashire who have special educational needs and/or disabilities.

In August 2023, we reviewed our approach for getting feedback and replaced the Annual Review survey with the  SEND survey.

We think the SEN survey is better because:

  • It focuses on what children and young people will need in the future as they become adults
  • The results from the survey are used to improve services
  • It can be changed quickly to look in more depth at particular issues, and we can then do additional work with children, young people and families via focus groups
  • It can be accessed via a smartphone and other devices
  • It is a short survey which is easy to complete and so more people will use it

These surveys are for children and young people and their parent carers in Lancashire where the child or young person has special educational needs (SEN) support needs.

All responses will be anonymous. It is your choice to answer any question or skip it. These surveys have been designed by Lancashire SEND Partnership to help us listen to children and young people (and their families) with different kinds of SEN. We want to understand what is important to children, young people and their parent carers to help us work more effectively with you.

Sometimes a situation might not have improved.  It is very important for us to know about this so that we can get in touch with the services to make changes and/or arrange support in a different way. There are text boxes included at the end of the survey for you to provide more information to tell us about this.

We will continue to use feedback to adjust and change the survey so that we can make improvements that we hope will in turn improve outcomes for children and young people. We'll publish the feedback from these surveys on the SEND local offer.
