Self referral
Our easy to use online system now enables you to work out if you, your friend, or relative, might be eligible to receive social care support. After you complete a few questions, you will receive an instant response telling you whether you are eligible to receive our services. If this is the case, you will be referred to a member of our customer care team who will process a formal care assessment to advise what support is required to meet your needs.
This service is only available for adults over the age of 18, living in the Chorley, South Ribble, West Lancashire (Ormskirk, Skelmersdale, Kirkham and Preston), Burnley, Pendle, Rossendale, Accrington and Rishton areas who are finding looking after themselves increasingly difficult.
