There are several ways you can order a copy death certificate:

• Online - request a copy certificate
• By telephone 8am - 5pm Monday - Friday on 0845 0530021
• By mail: postal applications to:

• In person at the above address 9am - 4pm Monday - Friday (10.30am first Wednesday every month)

  Please be aware when applying in person as there may be a waiting time whilst the certificate is being produced

Cheques or postal orders should be made payable to Lancashire County Council.

• Full name at Death. If a married woman please give name and surname of husband
• Date of birth
• Occupation
• Place of Death - (full address or name of hospital etc.)
• Home Address
• Date of Birth or Age at Death

The certificates issued are standard certificates. These certificates are a certified and complete copy of the original entry of the record of the death.

If the death occurred recently you may order copy certificates from the office where the death was registered for £4 otherwise certificates cost £10.00.
